@Incero can do it for $415/mo with $700 setup in their Giga Plan
Wow, their IP address is really cheap. Any other provider that can offer IP that cheap?(except ubiquity)
Most of our customers (vps providers) on here contact us for a custom quote in order to pay a higher setup and get the monthly fee down to ~$200.
Other than Incero, a few other providers that come to mind are: QuadraNet, FiberHub, and SecuredServers. -m.

Colocrossing can do for really if you purchase big servers
QuickPacket is another fine dedi provider.
We have Dual L5649s (12 CPUs w/ HT) in Bloomington, IL. These servers make REALLY nice VPS nodes and we can set you up with 1gbps unmetered for a cheap price, one issue though: We use HP Raid Cards and not LSI so we can't provide the LSI CadeCache module.
