iirc, IPv6 not available on ioflood, and IPv6 is a strict requirement for NodePing.
Maybe contact PhoenixNAP? Not sure about IPv6 though.

We tried them and found the support and service wanting. Our dedicated box was down for more than 8 hours with no reply from support.

Looks like @WiredBlade offers dedis out of Phoenix as well. Might be worth reaching out. The E3 listed on their site is slightly out of budget so they may be able to cut you a deal. Looks like they have good peering as well.
I know Leaseweb has stuff in Phoenix as well - however, they do require a justification for a /64 which is a bit.. weird

I put this together to help you out from reading the thread.
Leaseweb didn't work out. They require that I email them all kinds of verification documents.
I don't have problem providing documents if the company will provide a secure way for us to get those documents to them. Leaseweb says they'll only accept them if they're emailed to them. Sorry, I'm not emailing photos of my ID and credit card to anyone.
So I'm still looking for a dedicated E3 in Phoenix with the above specs.
@NodePing talk to @RyanD at TSS. They can likely fill this request in PhoenixNAP and they do have IPv6.

Signing up for a box now. I'm curious how they get ipv6 into PhoenixNAP (who already told me they don't support ipv6) - some BGP tunnel wizardry perhaps?
AFAIK, they connect directly to transit providers and just use PNAP for colo.
