I utilize two methods.
One observium for graphs, and statistical information.
Two I use server status, which I built, to have a quick overview of the servers, which also sends me alerts during downtime or high load. https://github.com/Munroenet/ServerStatus
The reason I do this is so I can run them in different locations, which allows me to have double proof that something went down or had an issue.

Yep, but old good Cacti will do the job as well. Easy to be installed using apt etc..Saw few firms using OpManager as well, they have free version(FREE Edition - Monitor 10 Devices), Linux and Windows. You have op5 Monitor Free edition as well, not same soft as OpManager just names are almost the same and opennms.
