Check if adblocker or something is blocking your form. Also see if you've any error logs. Paste them to a gist and post links here. That will help others to understand your problem. You've 0 interest in solving this issue. Why others should PM you to solve your issue. Not to forget for free.

I had to reinstall wordpress to fix the bug...:-(
BTW I still have no clue why there was missing information?
Most probably PHP related error I guess.

It's a php error related to insufficient memory allocated on the server or a CSF firewall rule PT_USERMEM set too low. Happens sometimes when you try to convert an existing WordPress site from standalone to multisite.
I swear it's always memory limit or a bad plugin.

Oooh yeah plugins. Before performing an upgrade to a WordPress multisite installation, deactivate all plugins and switch to the default (updated) theme. Because some plugins can't work on a multisite or clashes with another in a multisite environment.
the wp_posts contains 500,000 rows ....meaning 500,000 posts...may be that's why?..I changed db and table and it started working again

when I took the project I did not realize the dumbass had 500,000 posts....I told him to get a dedi....and he was on a shared hosting....and was not willing to move to a dedi if not in india....:-(
