I use virtualmin / roundcube. Works perfectly for me, I've used it for years.
You really just need to set up an email server with imap and then use a panel with it, e.g. squirrelmail.

Try Kolab (http://kolab.org). A bit of a long road to setup, but once done, you have everything - webmail, address book, calendar, filestorage, notes, etc.

Why don't you go with https://mxroute.com? Could be less costs than hosting your own mailserver.

I'm trying to set something up for a project of mine.
Basically, I want it so that users can sign up their email address on their own, and their accounts have to be manually approved by me.
And they shouldn't be able to SEND mail, only receive mail to their email address, unless I give them permissions on their account to SEND mail.
By default they shouldn't be able to send mail.
I think you are going to need a custom solution for that..
You can try our Mailcow (formerly known as fufix) if you want a self hosted solution.
You are able to do that easily with IspConfig.
You can even limit how many email can be sent in an hour.
If I recall I think ISPConfig gives you the option to disable imap, pop3, and smtp functions on mail accounts.
You will just need to install roundcube to finish it off.
If really brave you can use Horde Webmail. Its a pain in butt to setup but nice when running.
I'd say Zimbra if you have a lot of RAM available.
