you can ignore this one. because that repo is for trusty (14.04) only.

have you do apt-get update again? and is it still same error? because it's not possible, since "E:Some index files failed to download.They have been ignored, or old ones used instead."
Just remove the old PPA repositories that you've added, then rerun the upgrade?

check file /etc/apt/sources.list or folder /etc/apt/sources.list.d for sources that might be added.. remove the ones which refer to the old ubuntu version and try again.
In general, there's no guarantee that PPAs (being unofficial) will support upgrades to new major versions of Ubuntu.

from my experience in the past, do-release-upgrade is failing because of apt and dpkg is not updated to latest one.
try to remove the old repos (as per suggested by @nobizzle), or just run apt-get update again.
then, run apt-get --only-upgrade install apt dpkg and do-release-upgrade
Ubuntu offers an easy way for their users to break their systems. People should know the implications of installing unofficial packages but Ubuntu developers think it's better to just make the process of doing so as easy as possible.
