Yes, you can ask server providers to speak with ATT or Verizon to get this done for you.
It normally has to be done on a yearly contract and also there are additional cross connect charges etc with this kind of a set up.

I don't know what they are now, but it ain't lowend! Even with DIA, you wouldn't get "residential" IPs. You'd have to bring in some sort of xDSL, xPON or cable circuit.
Yes, he will have to bring in the circuit, but providers wont connect it to a VPS, it will mostly have to be done on a dedicated server.
I agree rates are insane, but people do have such requirements.
