the giver creates a token within the control panel for the server he wants to get rid off and sends this token to the receiver who can put it into the control panel as well.
server then gets transferred as it is, done.
Just remember that the seller need to wipe the server and the buyer need to check the hardware. Normally Hetzner is doing that after a cancellation.
