Need a E3 or better server with at least 8GB RAM, HD nominal, 4TB bandwidth, require single ipv4 and /64 ipv6, Ubuntu within NYC.
Budget - let's say the new dedicated trial cap of $84/month is my target but I can slide above that if need be.
Because I know how our community thinks... Some statements to head off the pack:
Yes, NYC location is important for this particular server and application.
No, NJ or "Metro NY" aren't good enough.
No, I don't want to argue why Piscataway should work since it's <1ms from NYC (I've got enough servers there).
Please don't offer any locations other than those within New York City, any borough will do fine.

USDedicated offers exactly what you need at 59.99$. It hosts within Manhattan Coresite Data center NY1. Havent seen anyother so cheap till now for manhattan.
Hivelocity just launched in NYC and they're doing some promos, like $79 E3s.

I contacted them for a quote about a month ago and never heard back.
That sucks. I had quick contact with them and managed to haggle a great price on a dedi. Their network performance is insane. Very happy with them. Only downside I can remember is waiting 1-2 days for the server to get deployed.
From USDedicated website looks like they are NJ with fiber to Manhattan. You'll have to double check with them... Not sure if they meet your ultra strict requirements or not.

The "New York Metro" is a very common thing. I've seen it all over Jersey and even some old Buffalo hosts that claimed similar.
