I am looking for looking for 2 VMs, 1 each in US East Coast and US West Coast. Some of the conditions:
- not at ColoCrossing, QuadraNet, ServerHub/EONIX
- at most, quarterly payments, monthly preferred
- budget: no more than USD 2.50 monthly for US East Coast, no more than USD 5.00 monthly for US West Coast
- IPv4 (a must) and preferably, IPv6 addresses
The VMs will be used for web frontends (reverse proxies).
Is this possible? Is there anything that anyone will recommend?

BandwagonHost is also worth mentioning. They offer OVZ/KVM 512MB VMs in NY, LA, Phoenix and Fremont for $7-8/qr. OVZ VMs come with IPv6 support. I have been using them for years.
They only have CC and ServerHub as upstreams (Just FYI).

It's above your budget, but the point is you can deploy 4 IPv4 VMs within this budget. Take a look at http://impactvps.com/knowledgebase/getting-started-vdr-virtual-dedicated-resources-pool/ They are in Incero DC (Seattle, NY, Dallas), as I recall. All their VDRs are OpenVZ based.
That's correct, one of their LA locations is actually in QNET.
