IP:PORT remote port foward, iptables or other way?

How can i for example have this:

1.2.3.4:1111 -> remote ip and remote port (whole different server, the port 1111 in netstat -tulpen is not open)
5.6.7.8:2222 -> original ip and original port (on port 2222 is already running sofware with opened port)

when someone load 1.2.3.4:1111 it will request from 5.6.7.8:1111, is this possible? thx!
