Hello everyone,
I've searched around but couldn't find any good survey softwares that are self-hosted.
Google Docs is great but its not self-hosted. Could the members of LET kindly recommend me good survey softwares? (both free and paid)
Thank you!

Seen this used a fair bit,
https://www.surveymonkey.com/
https://github.com/LimeSurvey/LimeSurvey
This looks decent from a quick Google. @ATHK he wants something self-hosted

Install Wordpress + Survey plugin
Honestly use google docs it will email you the results and put them into a speedsheet which means you can do anything with the data and if thats not enough you can dump spreedsheets into mysql and if you cant do it from there your on your own.
Google Docs can be great for you, but isn't what OP is searching for.
Limesurvey if self-hosted is a must, otherwise surveymonkey.
This is great and free but not self-hosted: http://www.typeform.com/
This looks good http://survdapp.com/ but haven't tried it honestly.
Thanks everyone for the help!
@jhadley that is so nice!!! You've been a great help (as usual). Thanks!
You can check out interspire's "email marketer" software program. PHP/apache based and fully Self hosted, does email surveys and web based ones in addition to other marketing aspects. Not the cheapest option, but one of the more robust ones and combined with things like email drip campaigns can get surveys done at certain intervals in a client's life time (first survey at day 7, another survey with different questions auto sent at day 60 to check in after the honeymoon phase has passed, etc).
