Another good one is CGIProxy. It's not PHP though.

One of LET member @titanicsaled started a project to enhance phproxy, but I'm not sure whether it is still actively updated.
I've used both Glype and CGIProxy before, and I can vouch for both. I've found both much better than PHProxy.

Just be careful as a lot of hosts don't allow the use of public proxies
